With this form, I wish to exercise my rights in the context of GDPR.
I understand that this form will be shared with those who need to know for the purposes of executing my request.

To assist Sogeti to locate my personal data, I provide the following information:

* For your protection we can only execute Data Subject Right requests based on the personal data that you provide us in this request form – name, first name, date of birth and e-mail address.

I wish to exercise the following rights under the General Data Protection Regulations (GDPR):

a. the purposes of the processing of my personal data;

b. the categories of my personal data which are processed;

c. the (categories of) recipient to whom my personal data have been or will be disclosed;

d. where possible, the period for which my personal data will be stored, or, if not possible, the criteria used to determine that period;

e. Any available information as to the source of my personal data you are processing, if this was not collected from myself;

f. the existence of automated decision-making, including profiling you are using;

g. if my personal data is transferred to another country and if appropriate safeguards are taken relating to this transfer.

* Please note that the right to obtain a copy shall not adversely affect the rights and freedoms of others. Therefore, information that is captured in documents such as emails memo’s etc. of other Data Subjects will not be send to you without the other involved Data Subject’s approval.

I have the right to obtain from Sogeti the rectification of inaccurate personal data concerning myself or to have incomplete personal data completed

I have the right to obtain from Sogeti restriction of processing because

a. the accuracy of the personal data is contested by me and I want to exercise the right to restriction of processing for a period enabling the controller to verify the accuracy of the personal data;

b. the processing is unlawful, I request the full restriction of use of my personal data. My personal data will not be deleted, however;

c. Sogeti as controller no longer needs the personal data for the purposes of the processing, but they are required by me for the establishment, exercise or defence of legal claims;

d. I have objected to processing and pending the verification whether the legitimate grounds of Sogeti override those of mine. The processing of my data should therefore be restricted.

I have the right to receive my personal data, which I have provided to Sogeti, in a structured, commonly used and machine-readable format and have the right to transmit or ask to transmit those data to another controller without hindrance from Sogeti.

I have the right to object, on grounds relating to my particular situation, at any time to processing of my personal data.

I understand that Sogeti, under the General Data Protection Regulation (GDPR), needs full proof of my identity before providing the requested information.
